Moreover, unlike insulin, metformin is not associated with hypoglycemic events. In severely burned patients, metformin not only improved glucose levels but also improved muscle protein synthesis. A recent study in burned adult patients confirmed the benefits of metformin treatment on glucose levels and insulin resistance. Similar to insulin, metformin reduced the post-burn inflammatory response. Insulin sensitivity was also enhanced with metformin administration. One contraindication of metformin administration in burned patients is the development of lactic acidosis. There has been one case report of lactic acidosis in a burn patient who was receiving metformin clinically for type 2 diabetes mellitus prior to injury. However, in a cohort of 18 metformin-treated severely burned adult patients, lactic acidosis was not observed. Studies are ongoing to evaluate the incidence of lactic acidosis in burn patients and whether metformin will continue to be a valid hyperglycemia treatment in this patient population. Preliminary data indicate that metformin is safe and effective in severely burned patients.
